WAIVER & RELEASE FORM In consideration of your accepting this entry, I, the undersigned, intending to be legally bound, hereby for myself, my heirs, executors and administrators and my dogs, waive and release any and all rights and claims for damages I may have against the “Walk With the Dogs” dog walk, Sonoma Valley Dog Owners and Guardians (SVDOG), and their respective agents, employees, volunteers, representatives, successors and assigns for any and all injuries suffered by me and/or any dog under my care in said event. I attest and verify that I am physically fit and that the dogs under my care in the event are well socialized and will be leashed and controlled by me at all times during the event. I hereby grant the sponsors permission to use my likeness and that of my dogs participating in this event without obligation to me. As part of this waiver and release I acknowledge that I have read and understand all of the above.
